
Person Centered Care (PCC) is at the heart of the devotion to competence and compassion in caring which distinguishes Cabrini Center for Nursing and Rehabilitation, (CCNR), established in 1992. The 240-bed CCNR offers24-hour skilled nursing care, short-term rehabilitation, post-hospital care and hospice services to a richly multi-cultural population reflecting the Home's diverse and vibrant East Village neighborhood in New York City. The PCC initiative promotes a more homelike, individualized, and collaborative environment. This continuing effort is based on a social model stressing the personal preferences of residents, staff and families and based on CCNRs core values.
Individuals in the community also benefit from CCNR's compassionate caring through Cabrini Long Term Home Health Care, Cabrini Care at Home, and the Sr. Josephine Tsuei Senior Day Services, a medical model adult day health center located on South Street. CCNRs sponsorship of Cabrini Immigrant Services, on Henry Street, further fosters the vision of St. Frances Xavier Cabrini, worldwide Patroness of Immigrants.
